Chapter 11 - Drew Had Changed the Record After the Accident
The investigation eventually established a sequence.
Not perfectly.
Enough.
At approximately 7:10 a.m., Caleb inspected Trench B.
He concluded the south wall was unstable after overnight water infiltration.
The trench shield had been pulled partially out the previous afternoon to allow equipment access and had not been properly repositioned.
Caleb wrote:
STOP WORK.
Crew members confirmed he said:
“Nobody goes down until the box is set.”
Then Caleb left that area temporarily to meet a utility inspector on another section.
During that time, Drew arrived.
Schedule discussion followed.
Drew told the superintendent they could continue preliminary grade work while the equipment operator repositioned protection.
Accounts differed on exactly what he authorized.
What became clear:
work resumed before the protective system was fully corrected.
Caleb returned.
He argued with Drew.
Witnesses heard enough to know:
they disagreed.
Then Caleb entered the trench.
Why?
That became the hardest question.
Drew claimed:
Caleb voluntarily decided conditions were acceptable.
One worker said:
Caleb went down to inspect the grade after being told the trench box had been repositioned.
Another believed:
he was checking whether the south wall needed additional correction before crew reentry.
Nobody could prove precisely what Caleb believed in the final minute.
The wall failed.
Afterward, chaos.
Then recordkeeping.
The 7:05 inspection entry stating:
protective system acceptable
had not been written by Caleb.
Digital metadata from a scanned document and handwriting review became part of the evidence.
Investigators concluded the log had been recreated after the accident using an older Caleb inspection as a model.
Who did it?
Drew eventually admitted directing the office to:
“clean up the daily sheet”
because the original field page could not immediately be found.
He denied ordering anyone to falsify safety conclusions.
But the replacement entry was inaccurate.
And it made Caleb appear to have approved the trench.
That mattered.
The original STOP WORK page had disappeared.
