Investigators determined the fire began inside an aging electrical junction behind the upstairs media-room wall.

The preexisting emergency plan that excluded me from shared protection.
The three-mask inventory despite a professional recommendation for four.
Bennett’s return for the emergency bag while I was still inside.
No single fact existed alone.
